In the latest issue of Seventeen, Modern Family star Sarah Hyland has dropped the bomb that she underwent a kidney transplant last month.

According to the interview, the 21-year-old cutie -- who revealed she has battled kidney dysplasia her entire life -- went under the knife on April 13, 2012, and received her new kidney from none other than her father.

"You know that family is always going to be there for you – no matter what," she said (via PEOPLE). "My dad gave me a freakin’ kidney! But it's also the families that you create outside of your family. And you really find out what kind of people you're friends with."

Sarah -- who says she's been given a "second chance" on life -- added that her boyfriend, actor Mark Prokop, has been a huge help in her recovering, admitting she is "one of the luckiest people in the world."

"He helps me with my medication and takes me to doctor appointments and all the stuff that I need to do to take care of myself. Any other 21-year-old guy would not be there, I guarantee it."

Modern Family airs Wednesday nights on ABC.

    I have been a loyal ABC morning news person since the days of David Hartman. When I saw Katie Couric sub for Robin, I was appalled! Katie does NOT fit the sophisticated-yet-fun news format ABC viewers enjoy. She is way too cutsie for me. Leave her to NBC or wherever she is from. Robin has nothing to worry about, that is for sure! If Katie appears again on Good Morning America, I will definitely change the channel!
